Ankle Pain in Active Adults: Causes, Treatment, and Prevention

Are you an active adult hobbling around with ankle pain? Whether you're a weekend warrior on the pickleball court, a dedicated trail runner, or someone who just enjoys a good walk in the park, ankle pain can really put a damper on your active lifestyle. As a physical therapist who's both experienced and treated ankle pain (yes, I'm a mess all over), I'm here to share some insights that could help you get back on your feet – pain-free!

What Causes Ankle Pain?

Ankle pain can stem from various factors, and it's not always because you tried to recreate your high school basketball glory days. Here are some common causes:

  1. Weak Foot Muscles: Those little muscles in your feet can have a big impact. When they're weak, your ankles have to work overtime.
  2. Limited Toe Mobility: Believe it or not, stiff toes can lead to ankle issues. It's all connected!
  3. Poor Ankle Range of Motion: If your ankle is stiffer than your dance moves, it can cause pain and altered movement patterns.
  4. Calf Muscle Tightness: Tight calves can pull on your ankle, causing strain and limiting your mobility.
  5. Unstable Hips: Hip issues can cause a chain reaction, leading to misalignment and uneven stress on your ankles.
  6. Overuse: Repetitive motions in activities like running or jumping can lead to wear and tear on the ankle joint.

Understanding these causes is crucial in addressing your ankle pain effectively. Remember, your body is like a game of Jenga – move one piece, and the whole tower can wobble!

Our bodies are remarkably adaptable, constantly changing in response to the demands we place on them. While this adaptability is generally beneficial, it can sometimes lead to problems. When we consistently use poor movement patterns or maintain certain postures for extended periods, our bodies adapt to these suboptimal states. This can result in some muscles becoming overworked and tight, while others become weak and underused.

For instance, a pickleball player with weak foot muscles might develop an altered movement pattern that puts excessive stress on their ankles. Similarly, a trail runner with poor hip stability might overcompensate with their ankles during uneven terrain. In both cases, the brain may have long ago decided to alter activation patterns in the muscles around the ankle as a protective mechanism, and other problems and pain start to surface with continued use.

These imbalances don't just affect the ankle itself. They can create a chain reaction throughout the body, altering our overall movement patterns and posture. Over time, this can lead to a lack of resilience - our bodies become so accustomed to these imbalanced patterns that we struggle to move efficiently in other ways when needed.

Our Holistic Approach to Treating Ankle Pain

At Activated Movement Physical Therapy, we don't just treat the symptoms; we look at the whole picture. Here's how we approach ankle pain treatment:

  1. Comprehensive Assessment: We examine not just your ankle, but your entire lower body mechanics, including your feet, knees, hips, and even your back.
  2. Muscle Energy Techniques: These hands-on methods help restore mobility to stiff joints and relax tense muscles around the ankle complex.
  3. Foot and Toe Rehabilitation: We address any weakness or lack of range of motion in the feet and toes. You'd be amazed at how much wiggling your toes can help!
  4. Be Activated Techniques: We use assessment and reflex work to address underlying muscle activation patterns that can be causing pain in activities.
  5. Targeted Strengthening: We design exercises to strengthen the muscles supporting the ankle, including the calf muscles and the often-neglected foot intrinsics.
  6. Hip and Core Strengthening: Improving hip stability and core strength can significantly reduce stress on the ankles. It's all about building a solid foundation!
  7. Mobility Exercises: Gentle stretches and range-of-motion exercises help improve flexibility and reduce stiffness in the ankle joint.
  8. Sport-Specific Training: We provide guidance on proper technique for activities like running or pickleball to reduce ankle strain, as well as provide a program to progress you to the amount of loading you need for your activity.
  9. Education: We empower you with knowledge about your condition and teach you self-management techniques. Knowledge is power!

A Personal Journey: My Experience with Ankle Pain

If you've read any of my other articles, you know by now that I'm practically held together with duct tape and optimism. Well, add ankle pain to my list of greatest hits!

My ankle troubles started during my college years after a terrible sprain during a pick up game. I struggled with continued pain all the way through physical therapy school 3 years later. Once I started to learn things that could help me, like learning to balance on the right leg again, strengthened my hips, and stopped wearing ankle braces every time I did any activity, I never had another problem that ankle!

Living with ankle pain doesn't have to be your new normal. Whether you're a trail runner, a pickleball enthusiast, a weekend warrior, or just someone who wants to walk to the mailbox without wincing, there's hope. With the right approach, you can overcome ankle pain and return to an active lifestyle, often with better form and function than before.

Remember, your body is adaptable and resilient. Sometimes it just needs some expert guidance to remind it how to move correctly. If you're tired of ankle pain holding you back from the activities you love, why not take the first step towards recovery? (Pun absolutely intended!)
